All services provided by Crystal Clear Concierge may be used for lawful purposes only. Crystal Clear is agreeable to providing services to the Clients, on the terms and conditions as set out in this Agreement.

Crystal Clear will ensure that all efforts are made to provide an estimate of the time required to complete a task and the approximate cost for products. These estimates are only approximates and may vary depending, but not limited to various external factors such as traffic, weather, third party charges, etc.

Service Usage
Membership is for the period of one month (30 consecutive days) or one year from the 1st hour of service used. Services not used by the stated time period are not refundable or transferable. Services may be substituted in any package by the client with different cost considerations. Please read all stipulations noted in contract for each service and or package. Vendor pricing and supplies are paid by the client, and fees are collected after order is placed before time of service. After membership hours are used, members are entitled to A-la-Carte pricing purchase of additional hours. Rush orders, weekend or holiday requests and after hour orders are billed at the Premium Rate of $80.00 per hr., (except where specifically noted). Rates are subject to change at anytime without notice.

MILEAGE POLICY
The first 5 miles of travel are included (except where specifically noted). Any Mileage over 5 miles is charged at $.45 per mile. Mileage may not be included in & is additional to any gift certificate time purchased.

Payment:
Payment is due immediately after date of invoice. All payments must be in US dollars and withdraw able from a US bank. The member is responsible for providing up-to-date payment information. Crystal Clear Concierge will not be held responsible should the member payment information become out-of-date.
Methods of Payment: Cash, MasterCard, Visa, Money Orders or PayPal accepted (must have a PayPal account). A small handling fee will be assessed for credit card payments.
If payment is by credit card, the member authorizes Crystal Clear Concierge to charge the credit card listed on the membership form for those charges for Crystal Clear Concierge services that may accrue from month to month, or for any outstanding balances, in order to bring the account to current status. Credit card payments will be billed and charged automatically, and Crystal Clear Concierge may charge the amount due to the provided card at any time.
If the Client will be using the Credit Card on file to place money into a Concierge Account they will have to sign and fax the Credit Card Authorization Form to Crystal Clear Concierge. Other forms of payment accepted are cash and money orders. Client will have their credit card charged immediately upon processing of products and completion of services. The Client will be contacted by Crystal Clear Concierge if Client's Credit Card is declined. Alternative payment must be arranged immediately. Any charge back fees caused or directly due to the Clients error will be paid by the Client and subject to a $50.00 fee.

Errors & Omissions for Referred Business Services:
Crystal Clear Concierge endeavors to provide services of the highest quality for all of its clients and will from time to time give Client referrals to 3rd parties for certain businesses. Businesses are referred by Crystal Clear Concierge, based on the professional reputation of each company, agency, business, service or individual and are expected to provide quality service. Crystal Clear Concierge is not liable for any unsatisfactory services provided by any referred businesses or for any claims arising from any referred business inability or failure to perform their agreed services in anyway. Although Crystal Clear Concierge may suggest a referred business, it is the sole responsibility of Client to select the business which best suits the Client's needs.
From time to time, Services provided to the Client will require Crystal Clear Concierge to enter into the Client household to complete Services. Residential keys may have to be provided to Crystal Clear Concierge Staff Member; this is done so in good faith of both parties. All Services will be carried out in a timely fashion and are entered into with both parties full consent.

Confidentiality:
Crystal Clear Concierge will not divulge to any third parties confidential matters or give out personal information without the Client explicit permission. Except where specifically agreed otherwise, all material, data, information etc. collected during the course of the Agreement will remain in the possession of Crystal Clear Concierge and not used without their permission. All employees of Crystal Clear Concierge have passed a national background check and have signed a Confidentiality / Non- Disclosure form; we comply with all standards of the Privacy Act.

Infractions:
Crystal Clear Concierge shall have the right to suspend service to a member at any time, and for any reason, without notice. If such a suspension is to last for more than 30 days, and the membership terminated the member will be notified as to the reason.

Cancellation policy:
Only a written request which must be received 30 days prior to termination of the Clients service relieves the Client of their obligation to pay the monthly account charge. Accounts in default are subject to a late payment charge of $25 and an interest charge of 1.5% per month on the outstanding balance. If the member defaults, the member agrees to pay Crystal Clear Concierge its reasonable expenses, including attorney and collection agency fees, incurred in enforcing its rights under these Terms and Conditions.

LIMITED LIABILITY. ANY LIABILITY OF THE COMPANY, INCLUDING WITHOUT LIMITATION ANY LIABILITY FOR DAMAGES CAUSED OR ALLEGEDLY CAUSED BY ANY FAILURE OF PERFORMANCE, ERROR, OMISSION, INTERRUPTION, DELETION, DEFECT, DELAY IN OPERATION OR TRANSMISSION, COMMUNICATIONS LINE FAILURE, THEFT OR DESTRUCTION OF OR UNAUTHORIZED ACCESS TO, ALTERATION OF, OR USE OF RECORDS, WHETHER FOR BREACH OF CONTRACT, TORTUOUS BEHAVIOR, NEGLIGENCE, OR UNDER ANY OTHER CAUSE OF ACTION, SHALL BE STRICTLY LIMITED TO THE AMOUNT PAID BY OR ON BEHALF OF THE MEMBERSHIP TO THE COMPANY FOR THE CURRENT MONTH.

Should any provision of this agreement be held to be illegal, invalid, or unenforceable by a court law, the legality, validity and enforceability of the remaining provisions of this agreement shall remain unaffected thereby unless otherwise stated. This agreement shall be governed by the laws of the State of Wisconsin and the United States. These Terms and Conditions supersede all previous representations, understandings or agreements and shall prevail notwithstanding any variance with terms and conditions of any order submitted. Use of Crystal Clear Concierge service constitutes acceptance of these Terms and Conditions.
